Thesis and Project Deadline Dates for the MSQA Program for Fall, Spring, and Summer Terms

Important Student Responsibilities:
Students are responsible for all information in this document.
Remember that in QAS 599 and QAS 600S the student works with his/her thesis/project committee members – Your Mentor is the thesis/project committee Chair.
Remember that QAS 599 is not offered during the summer term

Deadline Dates are AbsoluteRemember that deadline means the LAST day that you can turn your thesis/project in. There are no exceptions to these deadline dates. You can always turn your thesis or project in before these dates. If the stated deadline date falls on a weekend or holiday, the deadline is the workday before the weekend or holiday.

If you need editorial assistance with your paper you may contact your committee chair.  The MSQA may work with several editors who can assist with preparations for your thesis/project approval by the GSO. Please be advised that the University does not endorse individual contractors.

Once you thesis/project committee members have approved your thesis/project
You need to do the following now that the committee has approved your thesis/project.
1.  Fill out the Thesis or Project cover sheet to include all of your contact information. (Page 66, Thesis and Project Guide {TPG} see http://www.csudh.edu/graduatestudies/thesis/guide.shtml)
2.  Request your committee to sign the approval page and fax or email PDF file the signed signature pages to Rodger Hamrick at fax (310) 516-3753 or email rhamrick@csudh.edu.  At this stage each signature can be on a separate page.
3.  Use the four page checklist on pages 81 through 84 of thesis and project guide (TPG) to perform a final thorough check of your papers formatting.  A diligent effort will identify any remaining errors.
4.  Print a complete copy of your final thesis.  This should be on ordinary quality copy paper.  The 20 lb., 25% cotton bond paper will be used after the Graduate studies office review.
5.  Send the following to Rodger Hamrick in the MSQA Office
a.  The Thesis or Project cover sheet
b.  The hard copy project or thesis
c.  The completely filled out four page check list
d.  The IRB approval letter.

  • Evidence of enrollment in a course and payment of the course fee.
  • Must have applied for graduation
  • Must have submitted the Graduation Advisement and Advancement to Candidacy form filled out with student’s signature.
  • Must have taken and completed QAS 598.41, Directed Research.
  • Must have completed the Graduate Writing Assessment Requirement (GWAR/GWE).
  • Must have filled out “Thesis or Project Checklist” pages 81 through 84 of the Spring 2003 Edition of the CSUDH Thesis and Project Guide, “Purple Book”.  
  • Must submit the Thesis or Project Cover Sheet (found in Appendix A of the “Purple Book” {TPG} on page 66).  Your Student ID number must be on this form.
  • Must have signed signature page(s) from your three committee members showing that they have approved your thesis/project for submittal to the Graduate Studies office.  This may be on copy paper for first submittal to the GSO.

November 30th – Completed Signature pages due to the Quality Assurance office with three original signatures in black ink from the thesis/project committee members, minimum of six signature pages on white, high quality, 20lb., 25% cotton paper which contains a watermark. Students who are not in the USA need to send their signature pages to the MSQA office for signature processing.  All other students must complete this process themselves and see committee members for directions.  The Graduate Studies office will review the signature page for correctness.  Final approval of your thesis/project will be made by the Graduate Studies office only after it reviews your final signature page.
December 3rd - Students who do not present their thesis or project by the deadline, Friday, October 5th to the Graduate Studies office for review, must submit the work they have completed so far during this term no later than December 3rd at their course (QAS 599 or QAS 600) Blackboard Discussion Board in order to document your progress in completing this process.
December 7th - Final deadline date is December 7th for submission of the thesis/project with correct signature page, UMI publication forms and payment by check or credit card to the Quality Assurance office for submission to the CSUDH Library for publication.  Must submit with the original copy of the thesis/project the following:

  • Thesis and Project Approval Form that has signed approval by the Dean of Graduate Studies.  (We will Have this)
  • The Library requires that you provide an original copy of your thesis or project on 25% cotton content bond paper.  This copy will be used for publishing your thesis or project through ProQuest Information & Learning. The copy approved by the GSO.
  • There is a publishing/processing fee of $75.00 at time of submission of your thesis and supporting documents to the Library. The same copy will later be bound, cataloged, and available for checkout in the University Library. 
  • The supporting documents include completion of pages 4 and 5 of the ProQuest/UMI Dissertations Forms & Instructions for Paper Submission of your Doctoral Dissertation or Master’s Thesis. It is available at www.proquest.com/dissertationagree.
  • The user name is dissertations and the password is publish. The first page is ProQuest/Home/Dissertations. Scroll down and click 2010-2011UMI Dissertation Publishing Paper Submission Agreement (PDF). Print the Forms and Instructions.
  • Complete forms. Check Master’s Thesis in the upper right hand corner of the Dissertation/Master’s Thesis Submission Form, page 5. You must also check one of the boxes in Section III, Publishing Options & Signature, on page 4 (Traditional Publishing or Open Access Publishing) to let ProQuest know how to reproduce and disseminate your work.  The Open Access Publishing is optional for an additional fee of $95.  
  • Be sure to include a letter to Elizabeth Davis detailing your payment so that she will know what the total amount of check or credit card payment is for.  Without the detail she will reject your submission to the Library.  See example letter below.

As per my email to you dated September 17, the 2010-2011 the revised Publishing Agreement (UMI forms) is now available on line at www.proquest.com/dissertationagree effective the Fall 2010 semester.  Because of their change, attached is the revised “Library Form Letter to Thesis Students” including instructions on how to access the UMI forms and the fees, which have not changed. Please provide your students that will be completing their thesis/project the Fall 2010, Spring and Summer 2011 with the revised information.

Bring with you the Master’s Thesis Submission Form, the original thesis or project and the Thesis and Project Approval Form that is signed by the dean of Graduate Studies. Although optional, copyright service is available through ProQuest for a fee of $55.00. Print and complete the Copyright Registration Form on page 6 and submit it along with the thesis or project and above documents to the Library.

In addition, if you would like personal bound copies of your thesis or project, you must provide the Library with the copies and a fee of $18.00 per copy for binding. See below summary of costs and a sample of your total cost if you request copyrights and five personal copies.  You may also order personal copies from ProQuest (see Copy Order Form, page 7). The University Library will submit said order form along with depository copy to ProQuest.
