Manager and supervisor responsibility for an injured employee

Supervisors and managers must report all employee or volunteer work-related injuries or illnesses arising out of and in the course of employment to Human Resources Management at (310) 243-3771 within one working day of notice or knowledge.

If an injury or illness requires medical treatment beyond first aid, the Employee/Volunteer Report of Injury/Illness and Workers' Compensation Claim Form (DWC-1) must be provided to the injured employee within one working day of notice or knowledge. You are responsible for completing the Supervisor's Report of Injury and Illness form and submitting to Human Resources Management.

We are required to report Workers' Compensation injuries to our third-party administrator within five calendar days of knowledge or notice of a work-related injury or illness has been reported by the employee. Delays in completing these forms can result in delays in treatment and potential workers' compensation benefits to the injured employee.

Please encourage all employees, including student assistants and volunteers, to report any work-related accidents and/or near miss incidents when they occur. Timely reporting assists in identifying campus and safety issues and in determining eligibility for benefits. 

Note: First aid injuries where treatment is sought in the form of first aid kits in the department do not need to be reported to Human Resources Management.

Who is covered?

Workers' compensation coverage is extened to employees, volunteers, student assistants or students working in a work study program. For more information, please visit the following page "Who is covered."

Authorization for Medical Treatment

If medical attention is necessary, the employee may only receive medical treatment from one of the two approved medical centers for CSUDH, Kaiser on the Job or Concentra Medical Centers.

If you believe that the employee would pose a risk to himself/herself or to others if he/she drove themselves to the medical center for initial treatment, inform the employee not to drive. Arrange transportation with CSUDH Parking Services or if the employee has agreed to received treatment from Concentra Medical Center, contact Concentra Medical Center  to arrange transportation to the medical center. This service is only provided on the first day of treatment. All subsequent transportation needs must be arranged by the employee.

An employee who needs medical attention may not be transported by another University employee, supervisor or manager.

Important OSHA Requirements

Supervsiors MUST report all work-related deaths and serious injuries or illnesses immediately (within 8 hours) to Human Resources Management and RM/EHOS. Reporting delays could result in Cal/OSHA fines.

A serious injury or illness is one that requires inpatient hospitalization for a period in excess of 24 hours for other than medical observation or in which an employee suffers a loss of any member of the body or any serious degree of permanent disfigurement.

Notice of Return-to-Work

For all injured employees that have lost any time due to an industrial injury or illness, a notice releasing them to return to work is required before the employee can begin work. This notice must be sent to Human Resources Management.

All lost time must be verified in writing from a treating physician's report of injury, also known as a work status report.

If an employee is released to return to restricted or modified work, please contact Human Resources to determine whether light duty or modified work is available.
