OTD Early Application Review

• Attachments: Submit the following as separate PDF documents:

• • Application Checklist & OT Application Form.
  • 1-Page Resume
  • Proof of 80 Hours Volunteer/Observation Experience under a licensed occupational therapist OR Foundations of OT Workshop Certificate
  • Unofficial GRE Scores
  • Unofficial Transcripts reflecting the undergraduate GPA and grades of all prerequisite courses
    (Note: Up to a maximum of two prerequisite courses may be in progress)
  • OTD Information Session Certificate
• 3 Letters of Recommendation:
  
  • Letters of Recommendation need to be sent directly by the letter writers to otdadmissions@csudh.edu with the subject line: Letters of Recommendation for OTD Early Review
  • Note: 1 Letter of Recommendation from the licensed occupational therapist who performed supervision during the 80-hour Volunteer/Observation Experience and 2 from professionals who can attest to the applicant's ability for graduate coursework and level of professionalism
  • Note: If applicant completed the Foundations of OT Workshop in lieu of volunteer hours – the 3rd Letter of Recommendation is waived.
