Submitting form STD 261: Authorization to Use a Privately Owned Vehicle

(if applicable)

If driving a personal or rental vehicle, complete form STD 261- Authorization to Use Privately Owned Vehicles on State Business. 

• Initial: Employee completes Section 1 (Certification) and Supervisor completes Section 2 (Approval).
• Renewal: Employee & Supervisor completes Section 3 (Renewal) every year if authorization remains.
• Submit form STD 261 to Risk Management via Drop Box.
