Step 1: Submit an Interest Form
Access the online Interest Form to submit your responses. If a paper format is preferred, please contact your high school counselor to inquire about the TRIO UBMS Interest Form.
Step 2: Pre-Selection Interview
Once we have received your responses to the Interest Form, a Pre-Selection Interview will be conducted either in person or over the phone. This will allow us to determine your candidacy to proceed with our application, which is a lengthy process.
Step 3: Complete Application
Applications will be mailed out to all eligible participants upon the completion of the Pre-Selection Interview.
All applicants must complete the entire application packet and include all required supporting documents. The following items should be completed/provided prior to submitting your application to UBMS:
- Student Needs Assessment Survey
- Parent/Guardian and Family Assessment
- Student Application
- Release of Records Agreement
- Teacher & Counselor Recommendation Form
- Copy of School Photo ID
- Copy of U.S. Birth Certificate
- Copy of Social Security Card
- Copy of Current Transcript
- Copy of Parent/Guardian Current Form 1040 Income Tax
I've applied, what's next?
Please allow UBMS 2-3 weeks to review your application to ensure that all required documents have been submitted and that you have met eligibility. A UBMS staff member will contact you if additional information is needed.
Once selected, all new participants will be required to attend a mandatory New Student Orientation.
