STUDENT ASSISTANT/WORK STUDY INFORMATION

This section contains all payroll forms and information for DH student employees. General Questions regarding employee payroll should be sent to payrollservices@csudh.edu or by calling (310) 243-3769. You may also contact your Payroll Technician for assistance. Note: Forms containing social security numbers should not be submitted through e-mail. All forms must be signed and submitted to Payroll Services via the Payroll Services Secure Dropbox or deliver to WH 205.

Employee Information/ Payroll Changes

Student Payroll Action Request (SPAR) Form (std 457) used for:
- Designee Change
  - Instructions for Designee Change
- Withholding Allowance Changes
- Address Changes
  - Address changes can also be done through the myCSUDH self-service portal in the tabs for “My Personal Info – Maintain Personal Information – Change home/mailing address.”
- Name Changes or
- Birthdate Corrections
Direct Deposit Form-Enroll through Cal Employee Connect (CEC)
- Guide to enroll in Direct Deposit Program in CEC
- Instructions on how to register for CEC

Forms should be submitted to Payroll Services located in Welch Hall Suite 205 or via the Payroll Services Secure Dropbox.
