Drop, Petition, and Other Academic Forms

• Drop Online Courses Form:
  To drop online courses after the session has started **Not a Registration Form** This form is only to be used after registration has closed.
• Petition for Exception Form:
  Required form if you wish to appeal a University policy.
• Course Grade Appeal Form:
  Policy and forms to appeal a grade.
• General Education Petition for G.E. Course Substitution Form:
  For students seeking to substitute GE courses.
• Petition to Waive the Graduate Writing Assessment Requirement Form:
  For students seeking to waive the GWAR.
• Undergraduate Request for Planned Educational Leave Form:
  For students seeking to take an education leave must submit to the OLS program for signature.
• Change of Catalogue Year Undergraduate Form:
  For Students seeking to change their major to OLS and were admitted before Fall 2022.
• Change of Major/Minor Form:
  Required to declare your track (for Applied Studies students) or to change to the new major name, Organizational Leadership Studies. For students who were admitted prior to Fall 2022, you will also need to submit a change of catalog year in your final year of attendance.
• Satisfactory Academic Progress (SAP) Appeal Form: 
  Appeal form to the Financial Aid Office.
• Special Action Drop Courses Form:
  Request to add/or drop courses not in the OLS online course schedule or to enroll beyond 18 units.

Graduation-Related Forms

• Graduation Application Date Change Form:
  Students who are seeking to change expected graduation semester must officially request graduation date change.
• Late Graduation Application:
  Graduating students who did not submit their application online and have missed all deadlines must submit a paper application and a petition for exception form to the OLS Program Department.
• Request for Graduate Certification Form:
  Students who are seeking an official certification from the University regarding their graduation status for employment or other purposes.
• Request for Emergency Graduation Clearance:
  Students who need to have their degrees conferred faster than the normal processing times. Students must be able to provide documentation to the need for expedited service.
• Diploma Reorder Form:
  Students who wish to have multiple copies of their original diploma may purchase additional diplomas for $15 per diploma.
